Providence PACE
Rose Elder Law is pleased to offer information about the Providence PACE program to help families understand a care option that's often overlooked, and highly relevant to the retirement, long-term care, and Medicaid spend-down planning we do every day.
What is PACE?
PACE (the Program of All-Inclusive Care for the Elderly) is a federally recognized program that combines health insurance and hands-on care coordination into one team. Providence PACE brings together primary and specialty medical care, in-home support, transportation, adult day services, and more, all coordinated by a single care team built around the participant.
A common misconception is that PACE is only for people on Medicaid or only for those 65 and older. In reality, it's available to adults 55 and older who need a nursing level of care and live within the service area, whether they qualify through Medicaid/Medicare or choose to pay privately.
Who qualifies
- Age 55 or older
- Able to live safely at home or in a community setting (Assisted Living, Residential Care Community, Memory Care, or Adult Foster Home)
- Meet state service level criteria, generally meaning in need of support with mobility, cognition, and/or toileting
- Living within the Providence PACE service area: Multnomah County, parts of Washington and Clackamas Counties, Clatsop and northern Tillamook Counties
- Medicaid eligible, or willing to pay privately at the Medicaid rate
What's covered
- Primary and specialty medical care
- Dental, vision, hearing, and foot care
- Transportation and care coordination
- Medications, medical supplies, and equipment
- In-home care and home health care
- Adult day center services
- Physical, occupational, and speech therapy
- Hospitalizations, emergency, and urgent care
- End-of-life care
Things to know before enrolling
- Participants generally receive care through PACE's own network of providers, which may mean giving up existing outside doctors
- Care received outside the PACE program without authorization may not be covered, except in emergencies
- Enrollment is voluntary. Participants can leave the program at any time
Why this matters for elder law planning
Many of the families we work with are navigating exactly the kind of decisions PACE is built for: retirement planning, long-term care needs, and Medicaid spend-down strategy. Understanding PACE as an option before a health crisis hits can make a real difference in how a family plans, so we're glad to share it as a resource alongside our own legal guidance.
Enrolling in PACE through Medicaid comes with its own rules: financial eligibility under Oregon's OSIPM program, and Oregon's Medicaid estate recovery, which can reach Medicaid-paid PACE costs for participants 55 and older. Those are exactly the kind of questions worth asking an elder law attorney before applying, not after.
